Position Title: Youth Case Manager
Position Type: Full Time; 35 Hours Weekly

Salary: $40,000

Responsibilities:

• Participate in multi-disciplinary treatment team for collaboration of client care
• Provide direct care in accordance with treatment plan
• Engage all youth consumers in treatment planning and all areas of client care
• Create and adjust plans to re-engage non-participating youth
• Assist consumers with activities of daily living and obtaining basic needs
• Provide ongoing assessment of clients emotional wellbeing, program progress, behavioral changes, and follow ups
• Provide psychosocial education for consumers and family members
• Provide crisis intervention as necessary
• On-call duties as assigned
• Fieldwork required; agency vehicle provided

Requirements

• Bachelors Degree or CADC OR
• Associates with 2 years experience working with court involved services OR
• High school diploma with 4 years working with court involved services
• Minimum of 1 years experience working with youth in community setting
• Valid NJ Drivers License with less than 6 points
