We are looking for a Physical Therapist to provide one-on-one therapy care for patients in their home setting. Primary Job Responsibilities: Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met. Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.Utilize a variety of therapeutic modalities to maximize care provided to clientsRequirements: Vermont PT licensureHome Health experience and OASIS experience a must, as this position will case manage

Shift: DAYS: Monday-Friday (with weekend rotation approximately every 4th week or so)
