JOB TITLE: Community Services Worker III
CLIENT OR DEPARTMENT: County of Ventura | VCMC Utilization Mgmt
LOCATION: 300 Hillmont Ave, Ventura, CA 93003
DURATION: 90 days
SHIFT TIMINGS: Mon to Fri 8:30am-5pm & 1 weekend per month
BILINGUAL: Preferred

JOB DESCRIPTION:

Major Responsibilities: To assist licensed staff to coordinate post-acute care needs such as DME, transportation, home health services, hospice care, and skilled nursing facility placement.

Definition: Under immediate (I) or general (II, III) supervision, assists professional staff in providing transportation, referral, translation and program related services to culturally, economically, physically or mentally impaired or disadvantaged individuals in the community; and performs other duties as required..

Distinguishing Characteristics: This classification series is distinguished from other classifications by the support services

provided to disadvantaged individuals and groups within the community. Community Services Worker I is the entry level classification. Incumbents are expected to work in the trainee capacity leading progressively toward competency as a Community Services Worker II after one year of training. At the journey level, Community Services Worker II's are expected to function independently and effectively in performing all duties assigned. Community Services Worker III's differ from other classifications in that incumbents provide support or surveillance for minors/ dependents and elders under protective, custodial or probationary services requiring in-depth involvement with clients and their families and greater skill and depth of job knowledge in assessing problem situations and formulating plans for services.

Examples Of Duties

Duties may include but are not limited to the following:

  • Specific assignments vary according to the program assigned but may include all or a
  • combination of the tasks described below:
  • Answers inquiries and explains agency/program policies to individuals and groups within the community.
  • Provides translation and/or services, schedules appointments, purchases food and supplies for clients who are physically or mentally impaired.
  • Refers individuals and families to available community educational, medical, legal and employment resources; follows-up on referrals or appointments scheduled.
  • Drives County vehicles to pick up or deliver clients or supplies.
  • Keeps records of clients served, payments received and clinic activities; orders supplies and prepares routine reports.
  • Conducts community outreach to provide information on health related topics and services.
  • Keeps up to date on health program guidelines and requirements and assists in enrolling patients in programs.
  • Meets productivity and quality goals set by agency.
  • Addresses barriers to care by linking patients to internal and community resources.
  • Participates in care team huddles, meetings, and required training.
  • May lead group presentations and workshops on specialized topics including substance abuse, home management skills, job skills or health and nutrition to help individuals and families develop a healthier or more self-sufficient lifestyle.
  • May supervise clients during approved visitations or activities.

Qualification Guidelines

Knowledge, Skills, and Abilities:

  • Some to thorough knowledge of (depending on level within the series): agency and program policies and procedures; community structure and resources; group and family dynamics; County roads and buildings; problems, needs and behavioral patterns of the clientele served; and specialized topics for group presentation.
  • Some to working ability to (depending on level within the series): follow instructions; effectively communicate with individuals and groups from diverse cultural, social and economic backgrounds; transport passengers in a safe and timely manner; write case chronologies, field entries and other reports regarding clients; administer various health screening tests such as blood pressure, breath test, eye test; and work with physically/mentally impaired or economically disadvantaged individuals and groups.

Education/Experience:

Any combination of education and experience which has led to the acquisition of the required knowledge and abilities. The required knowledge and abilities also can be obtained by:

Community Services Worker I

One (1) year of public service experience in providing social services to the general public or equivalent experience/education.

Community Services Worker II

Two (2) years of public service experience or one year of experience equivalent to that of a Community Services Worker I.

Community Services Worker III

Three (3) years of public service experience or one year of experience equivalent to that of a Community Services Worker II.

Substitution: Fifteen (15) semester units of college course work in social sciences, ethnic studies, communications, or a closely related field may be substituted for six months of experience up to a total of one year.

Other Requirements

Some positions may require the possession of a valid California driver license
