Mission for Position Provide skilled nursing care and general health guidance to patients in the hospice setting so every patient and family receives personalized, compassionate care. Duties & Responsibilities Correctly implement the clientPlan of Care established by the physician Observe proper infection control and safety procedures Supervise health assistant services Coordinate care team to meet patient and family needs during end of life Position Type Full time, Monday-Friday 8a-4:30p, one holiday per year and call rotation one evening every other week and approximately every 8 th weekend.
