Touchpoints at Home an affiliate of Ultimate Care NY is a premier home health care company that has been serving the needs of Greater Hartford area and beyond for 10 years, is looking for a dedicated nurse to join their team as a Per Diem Nurse(link removed) This role requires a compassionate individual with excellent communication skills and a strong sense of responsibility.
As a Per Diem Nurse, you will be responsible for providing quality care to patients of all ages in their homes. Your duties will include administering medication, performing assessments, developing care plans, and providing emotional support to patients and their families.

Responsibilities
- Administer medication as prescribed by the patient's physician
- Perform patient assessments and develop care plans
- Provide emotional support to patients and their families
- Maintain accurate and up-to-date patient records
- Collaborate with other members of the healthcare team to ensure quality care
- Monitor and report changes in patient symptoms or condition
- Educate patients and their families on self-care techniques and treatment plans
Requirements
- Valid Nursing License
- Minimum of 2 years of experience in home health care (preferred)
- Proficient in Microsoft Office Suite
- Excellent communication and interpersonal skills
- Ability to work independently
- Strong critical thinking and problem-solving abilities
- Flexibility to work flexible shifts and weekends as needed
