Parx Home Care is seeking a skilled Per-Diem Home Health Physical Therapist in The Florida Keys area. Responsibilities
- Assess the patient's condition and develop an appropriate plan of care
- Deliver exceptional physical therapy services according to physician orders
- Evaluate the effect of treatment and modify care plan accordingly
- Educate patients and their families on proper home exercise programs and activities of daily living
- Maintain complete and accurate documentation of patient progress and treatment
- Work collaboratively with other healthcare professionals to provide comprehensive care
- Attend team meetings and participate in interdisciplinary clinical reviews
Requirements
- Current state license as a Physical Therapist
- Valid CPR certification
- A minimum of one year of experience in home health or clinical setting
- Ability to work independently and manage time effectively
- Excellent communication skills and ability to build rapport with clients
- Knowledge of Medicare and Medicaid regulations and guidelines preferred
