As a bilingual medical assistant, you will play a vital role in managing a caseload of patients while collaborating with our care team to provide exceptional healthcare services. Job Description:As a bilingual medical assistant in an office-based role, you will be responsible for supporting the coordination and delivery of patient care through: - Full-time Case Load Management: Collaborate closely with our care team to manage a caseload of patients, ensuring seamless coordination of their care.
- Chronic Care Management Calls: Assist in conducting chronic care management calls, providing essential support to patients managing chronic health conditions over the phone.
- Remote Patient Monitoring: Support Remote Patient Monitoring activities, helping to track patients' health data and ensuring timely interventions.
- Device Drop-Off/Pick-Up: Coordinate the drop-off and pick-up of medical devices for patients, facilitating their access to necessary equipment.
- Patient Alerts: Notify nurse practitioners promptly of any patient alerts or critical developments, enabling quick responses.
- Patient Visit Scheduling: Assist in scheduling patient visits, ensuring appointments are coordinated efficiently.
- Annual Wellness Questionnaires: Aid patients in completing annual wellness questionnaires, contributing to their comprehensive health assessment.
- Other Varying Tasks: Undertaking other varied tasks that contribute to the smooth functioning of the healthcare team
Requirements To excel in this role, you should possess: - Certification as a Medical Assistant (CMA) or Certified Nurse Assistant (CNA)
- Basic Life Support (BLS) certification
- Bilingual proficiency in English and Spanish to effectively communicate with patients.
- Proficiency in using electronic medical records (EMR) systems and other office software
