California Care Coordinators (CCC) is a Community Supports Provider contracted with Medi-CAL Managed Care Plans across Southern California. We assist the Unhoused to navigate the process of obtaining long-term sustainable housing via Rapid Re-Housing and Permanent Supportive Housing interventions. Your job is to help members obtain a Residential Lease Agreement so that CCC can cover the Deposits and other costs required to secure long-term housing. You will act in the capacity of an Independent Contractor contracting your services to CCC. This position consists of 3 stages:
Stage 1 - Housing Navigation: Work with members for up to 12-months to secure a Residential Lease Agreement and complete all items on the member's Careplan. Weekly report required. 35-50 member caseload.
Stage 2 - Security Deposits: Obtain Residential Lease Agreement and coordinate delivery of Security Deposits and other costs listed on the Residential Lease Agreement.
Stage 3 - Tenancy Sustaining: Work with member for a renewed 12-months after establishing long-term housing to successfully maintain tenancy. Complete any pending Objectives on Careplan. Monthly report required. Unlimited Caseload.
Responsibilities:
- Manage a portfolio of 35-50 Housing Navigation members ($135 per-member-per-month)
- Manage a portfolio of UNLIMITED Housing Sustaining members ($50 per-member-per-month)
- Your goals are to find members a RESIDENTIAL LEASE AGREEMENT and complete Objectives listed on member's Careplan
- Set your own consecutive 6-hour window as your "office hours" to complete tasks, make phone calls, make follow-ups and progress the member's Careplan. Office Hours not to exceed 9am-7pm window.
- Coordinate with property owners and landlords to secure housing options for clients
- Ensure compliance with all relevant housing regulations and policies
- Handle crisis management situations and provide appropriate interventions as needed
- Maintain accurate records and documentation related to housing services
Requirements:
- Experience working in housing management or a similar role
- Strong organizational and time management skills
- Ability to handle crisis situations calmly and effectively
- Excellent communication and interpersonal skills
- Proficiency with computers, files, attachments, emails, etc.
- Corporate etiquette and professionalism
This opportunity is perfect for case managers who are looking for work in a remote setting with massive earning upside in a rewarding field. You will also have the unique opportunity of running your own team of Housing Case Managers paid to you at $165 per-member-per-month, netting you $30 per-member-per-month, per Housing Case Manager on your team. A full caseload of 35 could net you $1,050 per each person on your team. Must demonstrate self-sufficiency and leadership skills along with a sustained period of no less than 6-months of excellent performance.
