Position Overview
Family Support Coordinator (REMOTE-Must Live in Virginia)
We are looking for a highly organized and compassionate Family Support Coordinator to join our team. As a Family Support Coordinator, you will be responsible for providing support and guidance to families who are experiencing difficulties in their daily lives that may lead to residential placement for their child. You will work closely with families to identify their needs and connect them with the appropriate resources and services.
Role Responsibilities
In this role, you will
- Connect families with appropriate community resources and services related to the family's needs and concerns.
- Assist with the development and implementation of individualized service plans for each family.
- Provide ongoing support and guidance to families upon service admissions and up to 90 days post-discharge.
- Collaborate with other professionals and healthcare providers, to ensure that families receive comprehensive support.
- Maintain accurate and up-to-date records of all interactions with families.
- Participate in team meetings and training to enhance knowledge and skills.
Basic Qualifications
- Minimum of 2 years of experience working with families in Behavioral Health or substance use disorders.
- Lived experience as a parent or guardian of a youth that has received behavioral health services (Preferred).
- Bachelor's degree in Human Services Field (Preferred).
- Strong communication and interpersonal skills.
- Knowledge of community resources and services.
- Ability to work independently and as part of a team..
- Strong organizational and time-management skills.
- Ability to maintain confidentiality and professionalism at all times.
