Case Manager - Transitional Housing and Maternity Housing
40 Hours Exempt.
Starting salary: $49,509.48
Hybrid Schedule 1 day a week

Position Description:
The full-time Case Manager for St. Margaret of Cortona Transitional Housing (SMC-TH) and Maternity Housing (SMC-MH) Programs of Catholic Charities of the Diocese of Arlington will provide intensive case management to clients at the St. Margaret of Cortona location in Prince William County. SMC-TH offers up to 24 months of transitional housing and services to homeless single mothers. SMC-MH offers housing and services for the remaining term of pregnancy at entry and up to 90 days after delivery for single pregnant women.
The Case Manager develops a strong professional relationship with the resident client population and must be able to relate to individuals making significant life decisions and be supportive of individuals increasing independence. The Case Manager maintains appropriate boundaries and ethics with clients and colleagues and assures confidentiality requirements are met. Must commit to utilize strength-based case management and motivational interviewing through a trauma-informed care lens. Must also be able to provide effective training, prepare written reports, and maintain client documentation in agency data systems. The Case Manager participates in decisions pertaining to Programming and client needs and cases in the absence of the Program Director.

Position Responsibilities:
Lead the intake screening interview with eligible referrals and recommend prospects to the Program Director for acceptance.
Knowledge of community resources for Inquiry calls.
Conduct client orientation for all assigned intakes to include a facility and unit description, review of policies, procedures, requirements, clients rights and responsibilities, grievance/complaint process and emergency disaster procedures and signing of the Program Agreement.
Assess clients physical, mental health and social needs, life skills competency, employability, educational needs, quality of life issues, strengths and needs, and other necessary areas.
In the Maternity Housing Program, capable of addressing birth plans with expectant mothers and discussing adoption services and making appropriate referrals regarding adoption and relinquishment planning, when necessary.
Conduct individual needs assessments, program planning, monitoring of clients progress, providing service referrals and coordination, client coaching, support, and training, from a strengths-based perspective, and assist with the process of finding permanent housing at program completion.
Provide intensive case management to residents who are on their path to independent living and financial stability, including education or training to increase earnings and reduce debt.
Maintain clear and objective documentation, enter data timely into the Client Management System

Other Duties:
The position will require some evening work and on-call responsibilities. The Case Manager will work closely with the Program Director, Art Therapist, Child Services Coordinator, Program Assistant and volunteers to holistically serve St. Margarets families.
Develop relationships with community and faith-based organizations, homeless services providers, medical/maternity practices, and any other partners within the Diocese as referral sources.

Qualifications & Skills:
Knowledge of Confidentiality, Privacy and HIPAA requirements and timely record maintenance.
Demonstrate skills in Motivational Interviewing, Trauma Informed Practices, and Boundary setting.
Knowledge of and experience with de-escalation techniques, including crisis-intervention, and conflict resolution.
Demonstrate productive time management, clear communication skills and ability to meet deadlines.
Objective and clear writing skills, produce case note documentation within 24 hours of meetings.
Basic calculation and statistical skills.
Capable of flexibility to include a hybrid office/remote work schedule based on client availability and program needs and Directors approval.
Knowledge of Mandated Reporting Requirements.
Willing to uphold Catholic moral & social teaching in the workplace.

Education and Experience:
Bachelors degree in social work or a related field required. Masters degree preferred.
Minimum 2 years experience working with low-income, moderate to high barrier women, including expectant mothers.
Bi-lingual English and Spanish preferred
Current CPR/First Aid certification.
Valid US drivers license, 3 years of driving experience in the US, and good driving record

Physical Demands:
Walking about the apartment facilities, climbing steps, and making home visits.
Indoor office environment with limited exposure to noise, dust, fumes, vibrations, and temperature variations.
Extended computer use, including time-sensitive email communications, data and documentation entry, report writing and online meetings.
Work pace is fast with unscheduled interruptions that may require immediate intervention.
Flexibility with work schedules based on client availability, occasional evening/weekend hours. Participate in on call rotation.
Access to transportation if agency vehicle is not available.
