Under the supervision of the Supervising Case Manager, the Case Manager (LPHA) will be responsible for the coordination and monitoring of a variety of services to meet the complex needs of Renaissance Treatment Center (RTC) clients. The Case Manager (LPHA) provides individual and group clinical care and treatment to chemically dependent clients. Ensures appropriate levels client support in various phases of addiction treatment while coordinating with therapists and administrative management on individual client treatment plans. The Case Manager will work closely with all RTC staff to efficiently coordinate referrals and identify access to services.
About the Facility:
The Renaissance Treatment Center, our 120-bed multi-dimensional residential treatment facility, addresses the complex issues facing individuals with substance abuse addictions by offering a unique continuum of care. This facility is in National City, CA.
Qualifications:
- Licensed (LCSW or LMFT) and at least three years of relevant work experience as an LPHA required.
- Must have a Master's degree in related field.
- Experience with mental health services, substance abuse, social services and health services preferred.
- Able to assist clients in recovery, treatment planning and updates, and exit planning.
- Strong written and oral communication skills.
- Bilingual in Spanish is a plus.
- Must be able to show respect, empathy, concern, and understanding to clients, the public, and staff.
- Must be computer literate in Windows and Microsoft Office.
- Must have valid California Driver's license.
- Must be able to obtain TB, drug screen and physical prior to employment.
- Must be able obtain CPR/First Aid within three months of employment.
