Summary

The Case Manager will play a crucial role in providing effective and efficient intake services for guests arriving at the facility.

Position Summary:

  • Compensation: $30.25/hour
  • Travel/Location: On-Site New York, NY

Requirements

Responsibilities:

Tasks, Duties, and Responsibilities

  • Responsible for assisting in the setup of the Intake area, register and provide bed assignments to all guests upon arrival to the Intake Center.
  • Provide all guests with a Guest Information and Rules handout and enter guest information into the HERRC Registration Application via the appropriate applications.
  • Confirm each guest signs the Policy Disclosure and Attestation Consent
  • Refer guest with illness, injury, or those with special medication needs (especially those needing refrigeration) and diet restrictions to medical staff
  • Direct all guests with pets or service animals to the designated pet area (if applicable)
  • Print and distribute guest badge and lanyard (if applicable)
  • If language interpretation is needed, utilize in-person interpretation services as available. If no interpretation services are available, request Language Line services from your supervisor.
  • Direct registered guests to the Reconnections Area for reticketing
  • Maintain a high standard to protect client information.
  • Provide exceptional customer services while assisting the client in coordinating necessary services and resources to address their sheltering needs.
  • Maintain client-related data systems, including case notes and a complete system of record entries.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities

  • Ability to effectively communicate both verbally and written.
  • Ability to maintain confidentiality with sensitive customer and internal information.
  • Capable of multi-tasking when necessary.
  • Strong familiarity with state and local governments is necessary.
  • Highly organized, detail oriented and self-motivated.

Experience/Education Required:

  • Bilingual proficiency in English and Spanish (includes reading, writing, and verbal).
  • One year of Case Management Experience
  • High School Diploma

Experience/Education Preferred:

  • Associates degree in social services
  • Fluency in other languages commonly spoken by migrants is an advantage.

Expected Hours of Work

Schedule may vary and require work on evenings and weekends and dependability onpunctuality are paramount.

Physical Requirements

  • May require sitting or standing for hours for a long period of time.
  • Mobility around client sites necessary.
  • May require long hours at a computer.Ability to lift and carry up to 30 pounds repetitively as needed.

Work Environment

  • Client site/Shelter
  • Work may require work in austere locations and various weather conditions.
  • All ACDC positions require travel to client sites or for emergency activations/deployments, as requested.

Other Requirements:

  • Must be 21 years of age or older.
  • Must pass company and any applicable drug, background check and reference check upon offer of employment.
  • Eligibility to work in the United States is required.
  • Ability to work onsite at and be present during scheduled work hours as required.
