Case Manager - Human Services Location: US-NC-Siler City Jobcode: f6cefb42383c74fa88eb752363bc9560-122020 Email Job

Provide crisis line and/or office coverage during the shift or project. Assess immediate crisis needs of clients and address appropriately. Manage incoming calls and case referrals and complete necessary documentation.
Responsibilities:
- Conduct potential placement client assessments and intakes.
- Complete and maintain client records and document services provided.
- Educate clients about options/resources available.
- Work with clients to develop and maintain relevant and meaningful case plans
- Meet regularly with clients to support case plan objectives.
- Refer clients to other agencies and resources and help client access necessary services.
- Facilitate problem solving for client conflicts and crises, encourage appropriate community interactions.
- Participate in staff meetings, case conferences and other meetings as needed.
- Support the daily operations of the organization.
- Perform other duties as assigned by Executive Director
Qualifications:
- A degree in Human Services or related field preferred, but equivalent work or volunteer experience and training will be strongly considered.
- Previous employment or volunteer work in human services, community agencies or other related nonprofit organizations a plus.
- Must be comfortable with and capable of working well with persons of varied cultures
- People-oriented - supportive and fairness - focused
- Good oral and written communication and computer skills are required.
- Excellent interpersonal skills and good organizational skills.
- Bilingual (Spanish/English) helpful. English must be spoken fluently.
- This position may answer calls and crisis during non-traditional work hours.
- Must have dependable personal transportation available at any time in the event of a client crisis. Must maintain a valid driver license, current registration, and auto insurance.
