Description

This position assists patients of the Mediciation Assisted Recovery clinic with their medical, behavorial, social, educational, and other supports to meet the individual basic needs. This position must provide services that are patient centered, individualized and culturally and linguistically appropriate to meet the individuals needs.

Examples of Duties

Support Direct Patient Interaction 100%
1. Work directly with clients who are seeking, receiving treatment, or have been identified as having substance abuse challenges.
2. Follows policies and procedures to ensure services provided are consistent with best practices and treatment goals.
3. Demonstrates therapeutic approach of being empathetic, respectful, and motivational, while empowering patients.
4. Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and clients.
5. Through skill building intervention and other supportive interventions, assists patients in effectively utilizing wide spectrum of resources, managing symptoms of illness and developing daily living skills to gain greater independence, employment and other meaningful activities.
6. Assists patient with coordinating appointment
7. Provide referrals to needed resources and monitor progress
8. Assist patients in accomplishing treatment goals.
9. Engages in advocacy for resource development.
10. Engages in advocacy on behalf of individual patients.
11. Demonstrates high professional and ethical standards.
12. Maintains confidentiality and security of Protected Health Information (PHI), outline in organization policy.
13. Additional duties as assigned by supervisor or program Director.

Qualifications

Any one, or any combination that totals 3 years from the following:

  • college course work comparable to that leading to a major in social work or one of the social or behavioral sciences or closely related field as measured by the following conversion table or its proportional equivalent:
    • 30 semester hours equals one (1) year (12 months)
    • Associate's Degree (60 semester hours) equals eighteen months (18 months)
    • 90 semester hours equals two (2) years (24 months)
    • Bachelor's Degree (120 semester hours) equals three (3) years (36 months)
  • progressively responsible social service/counseling work experience at a paraprofessional level under qualified supervision

Condition of Employment: Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
