Job description

Care Coordinator

Lolas New Beginnings provides PNCC/CCC services to all Medicaid eligible Pregnant Women and Women with Newborns who are interested in learning more about having a healthy baby. Our program serves residence throughout Milwaukee County.

Care Coordinators work with families to identify their goals, support families in efforts to meet their goals, provide information on how to self-care and care for baby as well as connect families with community services in their neighborhood/community.

Care Coordinators will accomplish these tasks by offering the services listed below:

PNCC/CCC services to families include:

Health assessments and information.

Goal setting and care planning

Advocacy

Provide referrals to community resources.

Free Carseat Program

Free Pack n Play Program

Free Diaper Program

Free CBRF Classes

Complete home visits

Take referrals

Provide health and nutrition classes

Provide parenting classes

Assist with concerns such as employment, education, W2, and WIC

We continue with coordination of services until the child reaches 7 years old

Requirements

  • Proven work experience as a Care Coordinator or similar role
  • Highly organized with a strong ability to manage different priorities
  • Ability to work with others and use resources to problem solve
  • Apply good judgment to fast-changing situations
  • Strong customer service skills
  • Effective verbal and written communication skills
  • Medical assistant certification preferred
