Care Coordinator
Place of Performance: Cottage Grove, MN

General Description:
Care Coordinator, assist the Case Manager by putting together and processing the different types of packets as well as enter and track patient information.

Specific Tasks:
Receive, communicate, and process the medical and personnel records that are submitted in all the various e-systems.
Travel to Camp Ripley, MN or other locations in the state during the performance of this contract to attend meetings, conduct training or Soldier Readiness Processing (SRP) events as determined by the needs of the state and individual units.
Generate reports and updates notes.
Maintain and verify medical provider credentials and privileging.

Experience:
Six years experience or equivalent experience, knowledge and/or abilities.
