6 month assignment, not confirmed yet if there are extensions 100% Remote, Shifts are 8:30 AM 5:30 PM (1 hour lunch) in the temp's local time zone No weekend shifts Job Summary The Care Coordinator, WTC-NPN is the primary point of contact for individual members and supports the member by taking a holistic, and supportive approach to the member's healthcare. The Care Coordinator is sensitive to the needs and characteristics of the unique population of Members serviced by the WTC-NPN. The Care Coordinator collaborates on an interdisciplinary team including RN Case Managers and Social Worker to meet the needs of WTC-NPN members assigned for case management services. Job Description Essential Functions Duties and Responsibilities Collaborates as an integral member of the WTC-NPN case management interdisciplinary team through clear communication and resource coordination to meet the shared goal of creating and nurturing a caring environment, supporting a better quality of life for the Member and reducing health care costs for the Program Assists RN Case Manager and Social Worker through contact with Members for assigned cases in scheduling appointments, arranging transportation, obtaining records, and documenting case management activities in case management software program. Assists RN Case Manager and Social Worker with follow-up actions to verify delivery of durable medical equipment or other supplies as ordered in the treatment plan for Member. Applies knowledge and experience in a healthcare environment as support to the case management team Notified case management team in a timely manner of identification of problems encountered by Member in obtaining services or supplies Promotes goodwill by assisting all involved parties on issues related to the coordination of the social services, as applicable Maintains privacy, as per Program guidelines, within a secure environment of documentation and communication Understands the organization's quality management program and the care coordinator's role within that program, with compliance of all SOPs within the Program's Operations Manual Is an effective team member; supports the team by pitching in at any level and effectively working across the organization to meet the needs of the case management program Takes ownership of personal actions and outcomes Embraces change; maintains an open mind and is flexible and adaptable in the face of ambiguity and change Ability to work independently, multitask, and adjust priorities Proactively seeks opportunities to increase knowledge, skills, and abilities May perform other duties, as needed, to meet the needs of the business Physical Demands The physical demands described here are representative of those that must be met by a colleague to successfully perform the essential functions of this job. The colleague holding this position must have strong communication and organizational skills, and an ability to interact and work with others. They must be able to work under time constraints and deadlines. 6 month assignment, not confirmed yet if there are extensions 100% Remote, Shifts are 8:30 AM 5:30 PM (1 hour lunch) in the temp's local time zone No weekend shifts Education High School diploma, Associate's Degree (or higher) in clinical or medical health administration, or related field preferred Experience 3+ years experience in medical record review, triage, customer service, prior authorizations, medical claims, and billing, assessing patient health, and/or related experience Communication Skills Excellent verbal, written, and interpersonal communication skills, organizational and analytical skills. Superior customer service skills working proactively and collaboratively with client colleagues and client leaders, medical staff, and co-workers. Ability to effectively present information. Excellent team player; ability to partner with internal and external clients and other cross-functional departments. Superior writing and communication skills. Must write succinctly and clearly and explain complex situations in plain English to technical and non-technical audiences. Utilizes direct communication and proactive problem solving to address challenges and disagreements. Reasoning Ability Ability to apply logical thinking to a wide range of intellectual and practical problems. Able to deal with very difficult concepts and complex variables. Ability to comprehend, analyze, and interpret documents. Ability to solve problems involving several options in situations. Requires intermediate analytical and quantitative skills. Other Skills and Abilities, Qualifications United States Citizenship Must be able to pass credit, education, and background checks Proficiency in Microsoft Office Coordination and Service Delivery Assess members unmet needs, coordinate services, and treatment needs Promote adherence to care plan, developed in coordination with the member, primary care, and other treating providers Appreciate and comply with all confidentiality requirements Be proficient in how to obtain and document an accurate medical history Be skilled in developing treatment plans with realistic goals Understand information systems and data management Physical and Psychological Factors Understand the physical and psychological characteristics of illness and be familiar with methods for assessing an individual's level of impairment Be capable of assisting individuals with the development of short- and long-term goals for recovery Case Management Concepts Be educated in current protocols and procedures for case management related to the WTC-NPN environment as related to the role of Care Coordinator Community Resources Access and evaluate available resources and develop additional resources as the need arises Maintain relationship with WTC-NPN case management team and other members of WTC-NPN team, as applicable to the role as Care Coordinator Conditions of Employment Other Requirements Ability to travel on occasion.
