Eminence Home Care MD is a leading provider of home care services in the Hospital & Health Care industry. Our mission is to provide compassionate care to our clients and their families by offering a comprehensive range of services to improve their quality of life. We are currently seeking a Care Coordinator to join our team and help us provide exceptional care to our clients.
As a Care Coordinator, you will be responsible for coordinating and managing the delivery of home care services to our clients. You will work closely with our caregivers and clients to ensure that our services are delivered with the highest level of quality and professionalism. You will be an integral part of our team, providing support and guidance to our caregivers and clients to ensure they receive the care they need.
Responsibilities
- Coordinate and manage the delivery of home care services to our clients
- Work closely with our caregivers to ensure that they are providing high-quality care to our clients
- Assess the needs of our clients and develop care plans that meet their specific needs
- Provide ongoing support and guidance to our clients and their families
- Collaborate with healthcare professionals to ensure that our clients receive the best possible care
- Maintain accurate and up-to-date records of all client interactions and services provided
- Ensure compliance with all relevant regulations and standards
Requirements
- Bachelor's degree in a healthcare-related field
- Minimum of 2 years of experience in a home health or home care setting
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of Medicare and Medicaid regulations
- Proficiency with Microsoft Office Suite
