PURPOSE: This scope of work is for CORE Project Management services to manage projects within the Director's office for the Florida Department of Health, Division of Community Health Promotion (Department)(link removed) The contractor will provide these services to the Florida Department of Health (Department).

5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE:

Contractor staff assigned to this agreement must possess the following minimum qualifications and experience:

5.1. Twelve years of project management experience.

5.2. Twelve years of experience communicating project updates both orally and in written form with technical staff, executive management, and user staff.

5.3. Ten years of experience with facilitating and developing applications from use cases process documents, and workflow diagrams.

5.4. Seven years of experience as Project Manager forstrategic Initiative/Special projects.

5.5. Five years of Contract and Grants Manager experience.

5.6. Five years of experience creating, updating and managing project plan to optimize resource utilization.

5.7. Five years of experience working with end-users and subject matter experts to document existing processes and identify appropriate changes to existing processes.

5.8. Five years of experience assisting in the development of performance reports and dashboards and assists users with the use of the Performance Management System.

5.9. Five years of experience with the use of Microsoft Visio, Word and Excel.

5.10. Five years of experience developing project plans.

5.11. Five years of experience in database analysis and design.

5.12. Minimum one year of experience working with Health Information Technology Surveillance Systems.

5.13. Five years of MS SQL Server experience.

5.14. Certified Project Management Professional (PMP).

6. CONTRACTOR RESPONSIBILITIES:

6.1. SERVICE TASKS:

Contractor will perform the following tasks in the time and manner specified:

6.1.1. Monthly, collaborate with staff and the Department'sstaff to establish short and long-term objectives, as directed by the Department.

6.1.2. Weekly, participate in regularly scheduled calls and meetings as directed by the Department.

6.1.3. Daily, ensure adherence to process and project management standards and guidelines (Department and PMO Policies and Procedures) as directed by the Department.

6.1.4. Daily, provide consultation to Department staff, and other partners, regarding ongoing projects and assignments, as directed by the Department.

6.1.5. Daily, develop and maintain project artifacts and application documentation as directed by the Department.

6.1.6. Daily, attend status meetings and maintain processflow documentation for projects as directed by the Department.

6.1.7. Daily, assist the Department staff with the coordination, scheduling and tracking of projects and assignments, as directed by the Department.

6.1.8. Weekly, develop and maintain weekly/monthly updates using MS Word, Excel, Project or other software tools or applications using project planning, as directed by the Department.
