CORE is a new program in our continued agency growth!

Responsibilities of the Core Clinician - The Community Oriented Recovery and Empowerment Services (CORE) Clinician is a member of the CORE Services Team providing clinical support and treatment to individuals with serious mental health disorders in their homes or in the community. The CORE Clinician is responsible for providing goal-directed supports and solution-focused interventions with the intent to achieve person-centered goals and objectives.

Specific duties included:

  • Provide home-based/community-based therapeutic interventions such as clinical counseling and therapy, which assist the individual in achieving stability and functional improvements. Develop and maintain trusting and supportive relationships with individuals, families, and community providers and supports.
  • Use a trauma-informed care and harm reduction approach to address behavioral health barriers that impact daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community participation.
  • Engage individuals in a person-centered Intake & Evaluation process to identify recovery goals, strengths and resources, barriers and needs, preferences for service delivery, and how the CORE service will be used to support attainment of their goals.
  • Partner in the development and implementation of strengths-based Individual Service Plans consistent the with individuals values, culture, beliefs, and goals. Updated Individual Service Plans on an on-going basis.
  • Provide support and treatment focusing on individual and family psychotherapy and counseling, integrated co-occurring disorder treatment, crisis prevention, and intensive rehabilitation counseling either one-on-one, or in groups.
  • Develop a working knowledge of CORE Services and practice according to the guidelines set forth by Office of Mental Health.
  • Coordinate and collaborate with the CORE Prescriber and Nurse.
  • Collaborate with all community providers including Managed Care Organizations, Health Home Care Management Agencies, Behavioral Health treatment agencies, and other CORE and HCBS service providers.
  • Provide transportation to individuals using agency vehicles. In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS drivers license, maintaining the NYS minimum insurance on personal vehicle if used, and must be approved by the agencys vehicle insurance carrier. An employee must always drive in a safe manner and use all safety devices.

Requirements

Skills/Qualifications for the CORE Clinician include:

  1. Must possess one of the following:
    1. New York State Licensed Master of Social Work , or
    2. New York State Licensed Mental Health Counselor
  2. One year of experience working in a behavioral health setting.
  3. Must have a strong commitment to the rights and ability of each personal with a mental illness or substance use disorder to make their own decisions and choices, live in the community, work, and have access to continuous support and services to enhance wellness and recovery.
  4. Must be approved to drive agency and personal vehicles for business purposes by: maintaining a valid NYS drivers license, maintaining the NYS minimum insurance on personal vehicle if used, and must be approved by the agencys vehicle insurance carrier.

Benefits

Schedule, Salary and Benefits of the CORE Clinician:

  • Full Time - Monday-Friday, 8:30am-4:30pm with some flexibility based on the served individuals needs, 40 hours per week. $67,160/annual equivalent.
  • Part Time - $32.00/hour, flexible schedule.
