Aleph Home Care is a Department of the Jewish Federation of Southern NJ is seeking a part-time to full time registered nurse to join our award-winning team of professionals. Aleph Home Care provides the highest quality personal care services in the home, while providing family the assurance that their loved one is being compassionately cared for.

 

Registered nurse position duties will include, but not limited to:

• In home nursing assessments, including the development of a plan of care suitable to the client’s personal care needs

• Supervise CHHA’s in the field while providing ongoing CHHA training and education

• Perform client reassessments in the home at least every 60 days or as needed relative to their medical condition

• Provide consultation to CHHA’s during regular business hours

• Represent Aleph in a professional manner in varying community settings

• Efficiently communicate with Director of Nursing regarding any changes in client’s condition

• Communicate professionally and effectively with Aleph’s office staff and management team

• Thoroughly document in client’s EMR in a timely and accurate manner

• Complete excel spreadsheets as requested for data collection and utilization review

Qualifications:

• Registered Nurse in the State of New Jersey, preferably at least one year

• BSN preferred

• Home care experience, preferably at least one year

• Geriatric experience strongly preferred

• Strong organizational and communication skills

• Proficient in Excel and Word
