Job title: Healthy Families Bilingual Family Assessment Worker Hospital Outreach Learn more about Healthy Families Job description: This role provides the first introduction to Healthy Families to prospective and new parents, as well as connecting with those community partners, at hospitals throughout Maricopa County. You will engage with families, in a hospital-based setting, as well as provide community outreach to cultivate strategic partnerships within the hospitals as well as a variety of organizations and agencies that serve families. In addition, this position provides screening and assessments to pregnant women and families with newborns who have been identified as having risk factors. Program details: Healthy Families is a home visitation program that supports pregnant women and families with newborns meet the challenges of parenting. The program is offered to families who are expecting a new baby or have a baby who is less than 90 days old. Services can continue for up to five years after the birth of the child. In this role you will: Introduce the Healthy Families program bedside in the hospitals and incite interest in the program by conversing with the families and assessing their wants, needs, strengths and impending challenges for their changing family. Establish trusting relationships with families quickly, while considering a families personal experiences and diverse cultural backgrounds. Engage in face-to-face contact with community partners in hospital settings, clinics, and elsewhere, providing presentations and maintaining positive working relationships to foster further collaboration. Promote community awareness regarding Healthy Families program services and developing relationships with community providers through community outreach activities in order to secure referrals. Conduct enrollment visits in a familys home when not schedule allows outside the hospital setting. Compose accurate and complete in-depth screenings, assessments, program intake forms, and data tracking within appropriate timelines. Provide comprehensive data, recording-keeping and reporting of community outreach efforts. If you don't think you can meet every qualification below, we still encourage you to apply. We value both current experience and future potential What it takes: Bachelors degree preferred. Associates degree with experience in early childhood (such as: home visitation, child welfare, prevention, child development, family dynamics) considered. Bilingual English/Spanish fluency required. Comfort, familiarity, or experience working in a hospital setting preferred. Knowledge of family dynamics and community resources preferred. Experience working with culturally and linguistically diverse populations. Strong communication and writing skills, along with the capability to work in Microsoft Office and Windows. Attention to detail with ability to perform assignments and job responsibilities efficiently, accurately and in a timely manner. Valid Arizona Drivers License Registered vehicle with valid auto insurance. Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card.
